Today I checked the ranking for my keywords, I saw some changes on my keywords.
On 10th July for "Medical Billing Company Pennsylvania" on search result it shows my home removed link but yesterday 27th July it shows removed link tag/medical-billing-company-pennsylvania/
Can anyone share me the exact reason for this URL change.

Do you have two webpages the home and the /medical-billing-company-pennsylvania/? If so Google chose the other webpage as a better page for that keyword.Today I checked the ranking for my keywords, I saw some changes on my keywords.
