What do you do for your local SEO clients once you eventually reach the point of ranking #1 in the city for all their service offerings?

After you have achieved #1 for one keyword or a set of them, then you start focusing on extending the reach of those keywords.  As well as finding more high-quality keywords and focusing on getting them ranking while maintaining your primary keywords.
